I have created this topic as it is of importance to the CURE programme and the future evaluation. Collecting patient stories will help us show our effectiveness from the patients perspective and also is great for promotion of our own services.
If possible collecting these patient stories that have been successfully treated through the service would be great!
I have attached a consent form for patients who would be willing to give there stories.
